    Bruxism is jaw muscle activity that may involve clenching or grinding the teeth during sleep or while awake. In some people it contributes to tooth wear, muscle overload and pain. Facial pain or headaches alone do not establish the diagnosis.

    Where should treatment begin?

    Dental assessment of the teeth, muscles and temporomandibular joints is important. Depending on the findings, management may include an appropriate splint to protect the teeth, awareness of daytime clenching, physiotherapy and assessment of stress or sleep-related factors.

    Snoring and witnessed pauses in breathing during sleep need investigation for sleep apnoea. Not everyone with bruxism needs injections.

    What is the role of botulinum toxin?

    Injected into selected jaw muscles, it temporarily reduces contraction strength. It may help symptoms associated with excessive muscle loading in suitable patients. It does not remove the cause of bruxism or guarantee that all clenching episodes stop.

    Treatment is considered individually, usually as an adjunct to conservative management. Botulinum toxin use for bruxism is off-label, which is discussed during assessment. Injections do not replace dental care or tooth protection.

    Consultation and injections

    Bring details of previous treatment, any splint, symptoms and medicines. The doctor assesses the muscles and contraindications, then selects injection sites and dose. The procedure involves several injections that may briefly hurt.

    The effect develops over days and weeks, then gradually wears off over subsequent months. Follow-up assesses pain, chewing function and adverse effects. Repeat treatment should not be automatic without reassessment.

    Contraindications and adverse effects

    Report neuromuscular conditions, swallowing problems, allergies and previous botulinum toxin treatment. Infection at an injection site or hypersensitivity to the product prevents treatment. Elective injections are deferred during pregnancy and breastfeeding.

    Bruising, pain, chewing weakness, an uneven smile and a change in lower facial contour can occur. Rare difficulty swallowing, speaking or breathing requires urgent medical attention. Repeated injections require weighing benefits against the risks of muscle weakening.

    Aftercare

    Do not massage injection sites, and follow your activity advice. Continue agreed dental care and physiotherapy. Persistent pain or difficulty chewing needs review.
