Monitoring Information Clause

    1. Pursuant to Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (OJ EU L 119 of 4 May 2016), we inform you that the Controller of video surveillance at the Tartaczna 2 Medical Center facility is: Centrum Medyczne Tartaczna 2 Sp. z o.o. Sp. k. with its registered office at ul. Tartacznej 2/1a, 80-839 Gdańsk, phone: 58 719 10 25.

    2. The Controller has appointed a Personal Data Inspector. For contact details, see iod@tartaczna2.pl.

    3. The surveillance data is collected and processed in order to protect facilities and property, and to ensure safety on the premises of the Tartaczna 2 Medical Center in Gdańsk, in the building at 2/1a Tartaczna Street. Video surveillance is carried out within the legitimate interest of the Controller. The data collected is processed only to the extent related to the fulfillment of the foregoing purposes.

    4. The Controller does not share monitoring data with other recipients apart from entities authorized by law.

    5. The Controller does not intend to transfer the monitoring data outside Poland or to international organizations.

    6. Surveillance data is stored for no longer than 3 months and is automatically overwritten. Data collection through the monitoring system at the Center's headquarters (ul. Tartaczna 2/1a, in Gdańsk) includes registration desk (video surveillance), entrance door, waiting room and the corridor leading to the office rooms.

    7. An individual may request the Controller to access, rectify, update their data, as well as have the right to restrict data processing.

    8. In connection with the processing of personal data in the form of surveillance by the Controller, you have the right to lodge a complaint to the supervisory authority, i.e. the President of the Office for Personal Data Protection.

    9. The Controller will not make automated decisions based on the collected personal data, including decisions resulting from profiling.

    10. The Controller does not intend to transfer your personal data to a third country or international organization.
