    The tear trough is the hollow extending from the inner corner of the eye towards the cheek. Shadow in this area can make the face look tired. In suitable patients, hyaluronic acid filler may soften the hollow and the transition between the lower eyelid and cheek.

    Will filler help dark circles?

    Dark circles do not always result from volume loss. Pigmentation, visible blood vessels, thin skin, swelling or protruding fat can also contribute. Filler does not remove under-eye bags and may worsen the problem in people prone to puffiness.

    The consultation assesses the eyelids and cheeks together. A different approach, or avoiding injections in this area, may be more appropriate.

    How should I prepare?

    Tell your doctor about previous fillers, eyelid surgery, persistent swelling, eye conditions, medicines and allergies. Pregnancy, breastfeeding, an active infection or allergy to an ingredient are reasons to avoid elective treatment. Do not stop medication yourself.

    What happens during tear trough treatment?

    The doctor places small amounts of filler in an appropriate tissue layer using a needle or cannula. Product selection, quantity and technique are particularly important around the eyes. You may feel the injections and pressure.

    The aim is conservative correction of the hollow. Overfilling does not improve the result and may cause unevenness or persistent swelling.

    Results and recovery

    The result is assessed after swelling settles, usually at follow-up. Duration varies, and filler may remain in this area longer than expected. Repeat treatment should not be scheduled automatically without examination.

    Bruising, tenderness, swelling, lumps, asymmetry and a bluish skin tint, known as the Tyndall effect, can occur. Report persistent changes to your doctor. Do not press or massage the area unless advised.

    Why is careful assessment important?

    The eye area carries an important vascular risk. Rarely, filler can block a blood vessel and cause skin necrosis or loss of sight. A cannula does not eliminate this risk. Sudden pain, a change in skin colour or any visual disturbance requires immediate medical attention.
