    ORGANISATIONAL REGULATIONS – INFORMATION FOR PATIENTS

    This document is an extract from the Organisational Regulations of Centrum Medyczne Tartaczna 2, setting out the main rules governing the organisation and provision of healthcare services, as well as patients' rights and obligations.

    1. Information About the Medical Centre

    The healthcare provider is Centrum Medyczne Tartaczna 2 spółka z ograniczoną odpowiedzialnością spółka komandytowa, with its registered office at ul. Tartaczna 2/1A, 80-839 Gdańsk, Poland, NIP 5833402512, REGON 386604641, KRS 0000851660.

    The Medical Centre operates under the Polish Act of 15 April 2011 on Medical Activity and is entered in the Register of Entities Performing Medical Activities (RPWDL), registration number 000000230464.

    The Medical Centre provides outpatient healthcare services within the scope of its RPWDL registration, the qualifications of its medical personnel and its organisational capabilities.

    2. Scope of Healthcare Services

    The Medical Centre provides healthcare services including, in particular, specialist consultations, diagnostic examinations, preventive healthcare and outpatient medical procedures.

    Services are provided by appropriately qualified and authorised healthcare professionals, in accordance with current medical knowledge, the duty of due care and respect for patients' rights.

    Detailed information about the available services and the current offer can be found at https://tartaczna2.pl.

    3. Payment and Financing of Healthcare Services

    The Medical Centre provides healthcare services on a fee-paying basis, in accordance with its current price list, as well as under agreements with other entities organising or financing healthcare services.

    Services may be provided, in particular, upon referral or commission from other healthcare providers, insurance companies, medical subscription and healthcare package operators, and other authorised entities.

    The scope of services covered by such agreements, patient referral and eligibility requirements, and financing arrangements are determined by the relevant agreements with cooperating entities.

    Patients are not required to pay the Medical Centre for services to the extent that their costs are covered by a cooperating entity. Services not covered by third-party financing may be subject to charges, of which patients are informed before the services are provided.

    The Medical Centre's cooperation with entities financing healthcare services does not mean that all services are generally available under such financing arrangements. Eligibility depends on the scope of the relevant agreement and fulfilment of its conditions.

    4. Registration and Appointment Organisation

    Patients may register in person, by telephone or electronically through the booking channels provided by the Medical Centre.

    Appointment dates are arranged taking into account the availability of medical personnel, the type of service and the patient's medical needs.

    Before receiving a fee-paying service, the patient is informed of its price, scope and any potential additional charges.

    Eligibility for a specific examination or procedure is assessed by an authorised healthcare professional. If medical contraindications are identified, the service may be postponed or not performed, subject to applicable law.

    5. Patients' Rights

    Every patient enjoys the rights established by applicable legislation, in particular the Polish Act on Patients' Rights and the Patient Rights Ombudsman.

    In particular, patients have the right to:

    • healthcare services provided in accordance with current medical knowledge and with due care;
    • respect for their dignity, privacy and personal intimacy;
    • clear and understandable information about their health, diagnosis, proposed diagnostic and treatment methods, and their possible consequences;
    • give or refuse informed consent to healthcare services, in accordance with applicable law;
    • access their medical records;
    • confidentiality of information concerning their health;
    • the presence of a close person during the provision of healthcare services, under the conditions established by law.

    6. Medical Records

    Medical records are generally maintained in electronic form, in accordance with applicable legislation.

    Medical records are made available to the patient, their legal representative, a person authorised by the patient and other legally entitled entities, within the scope and under the conditions provided for by law.

    Records are made available without undue delay, following verification of the requesting person's entitlement to receive them.

    The first disclosure of medical records to the patient or their legal representative within the requested scope is free of charge. Fees for subsequent disclosure of the same records may be charged within the limits established by Article 28 of the Polish Act on Patients' Rights and the Patient Rights Ombudsman.

    Current fees for accessing medical records are made available by the Medical Centre in accordance with applicable law.

    7. Cooperation with Other Healthcare Providers

    The Medical Centre may cooperate with other healthcare providers and diagnostic, pathology and genetic laboratories to ensure appropriate diagnosis, treatment and continuity of healthcare.

    Examination results and medical records are disclosed only to authorised recipients, in compliance with legislation concerning patients' rights and personal data protection.

    8. CCTV Surveillance

    The Medical Centre uses CCTV surveillance in selected publicly accessible areas to ensure the safety of patients and staff.

    Surveillance covers the entrance area, reception, waiting room and the corridor leading to administrative offices. It does not cover rooms where healthcare services are provided or sanitary facilities.

    CCTV surveillance records images only, without audio. Recordings are stored for approximately 30 days, subject to exceptions provided for by law.

    Detailed information about CCTV surveillance and the processing of personal data is available on the CCTV Surveillance – Privacy Notice page.

    9. Patient Complaints and Requests

    Patients, their legal representatives or other authorised persons may submit complaints or requests concerning the organisation of the Medical Centre, the quality of healthcare services or the observance of patients' rights.

    Complaints and requests may be submitted in person, in writing or electronically to kontakt@tartaczna2.pl.

    Complaints are reviewed without undue delay, taking into account the nature of the matter and applicable law. Where justified, explanatory and corrective measures are taken.
